Cloquet Public Schools uses online registration as the primary way to collect student information and required forms.
Most student forms — including emergency contacts, health information, permissions, and annual acknowledgements — are completed electronically during the online registration process. This helps ensure information is accurate, secure, and up to date for each school year.
Online Registration
Families should complete all required student forms through the Online Registration Portal at the beginning of each school year or when enrolling a new student.

Additional Forms (If Needed)
In some cases, a school may request additional forms throughout the year (such as activity participation, health documentation, or special program paperwork). When needed, these forms will be:
Provided directly by the school, or
Sent home with students, or
Shared via email or school communication platforms
