Medication administered at school (prescription and non-prescription) will need to be accompanied by a note from BOTH a parent and a physician. You can access a Medication Authorization form in the Medical Forms link at the bottom of this page.

Any self-administered medication such as epi-pens or asthma inhalers will also need to be accompanied by a note from a physician and parent indicating their approval for carrying the medication. There is a medication administration form specifically for this purpose.
