The School Immunization Law in Minnesota requires all students enrolled in grades kindergarten through 12 to show they have received certain immunizations or an exemption. Schools are required to report immunization data to MDH each year.

At a glance, which immunizations are required for school?

Before starting Kindergarten:

• DTaP - 5 doses

• Polio - 4 doses

• MMR - 2 doses

• Hep B - 3 doses

• Varicella - 2 doses

Additional vaccines needed before starting 7th grade:

• Tdap - 1 dose

• Meningococcal - 1 dose

Additional vaccines needed before starting 12th grade:

• Meningococcal Booster - for a total of 2 Meningococcal doses
