Students wishing to participate in Cloquet activities must complete the following:

1. MSHSL Eligibility Form – must be completed in Arbiter Registration

2. MSHSL Sports Physical – valid for three years from the date of the exam

3. Activity Fee – paid in full

Sports physicals must be completed and signed by a physician. Completed physicals can be:
